Before we come to your home, we'd love to learn a little more about you and your companion so we can make this time feel as peaceful as possible. We provide information to help guide you through this aftercare process.
This short questionnaire covers a few practical things — how your companion typically responds to vet visits and needles, any memorial keepsakes you'd like to have, and a few administrative decisions — so they're settled ahead of time.
That way, on the day itself, you're free to simply be present with your companion and each other, without having to think through logistics in the moment.
Memorial items: Urns, paw prints and keepsakes
Private cremation arrangements: If you have made private cremation arrangements for your companion, one of the five standard urns are complimentary. Engraving and personalization is available at an additional cost. You can also view personalized urns in the catalogue. Please provide your decisions in the questionnaire below.

Communal or private cremation arrangements: A selection of highly personalized keepsakes and memorial items such as digital paw prints, ceramic paw prints and jewellery are available at an additional cost. Please provide your decisions in the questionnaire below.

Please take your time answering the questions because we understand it can feel overwhelming. Your answers help us prepare for your time together to ensure you can focus on your companion.
A Meaningful Goodbye™
We encourage families to create meaning - possibly through a small act or gesture. It’s an opportunity to celebrate and honour your companion’s life and what they mean to your family. If you’re comfortable, some families have given special treats, played music, read poems or letters or had religious ceremonies.
In case of emergency
If an any point, it looks like your companion is in distress or mouth breathing, please urgently bring your companion to your family vet if they are open, or to your nearest animal emergency hospital.
